The MLSA Membership Application form is available as a Microsoft Word document download.

A blank application form should be printed off and filled out by hand. Completed application forms should be returned to Head Office and should be accompanied by the appropriate subscription amount or mandate form.

Applications received without the appropriate subscription or mandate will not be processed.
